What can I do if I need an appointment but my doctor’s schedule is full?
We try our best to work with patients who need to be seen immediately. If we can’t find time in your current doctor’s schedule, you may ask for the first available appointment with one of our other clinic doctors. We will do our best to see urgent problems within 24 hours. Please let us know if your problem is an emergency as you may need to go to the emergency room.
What should I do if I can’t make it to a scheduled appointment?
If you know you will not be able to keep your appointment contact our office as
soon as possible. This courtesy helps us provide timely care to all our patients. Cancellations should be made at least 24 hours prior to your scheduled appointment time. If you call less than 1 hour before your scheduled appointment, it is unlikely that the appointment will be available for use by anyone else.
If you cancel or fail to show for three consecutive appointments, we reserve the right to not reschedule your appointment.
Why do I need a referral to see a certain specialist?
Referrals are required by many, but not all insurance companies to ensure you are being referred appropriately. Insurance companies believe your Primary Care Physician (PCP) knows your medical history the best and can ensure you are seeing the correct provider to treat your condition. If your insurance carrier allows self-referrals, you may contact us directly without a referral from another doctor.
Can I still come to the clinic if I don’t have insurance?
Yes. However, those without insurance should expect to pay in full at the time of service. CORE offers cash paying patients a significant discount when paying at the time of service.
I am currently unable to pay my debt to the clinic, what should I do?
If may be possible, depending on the size of your family and income, for our staff to set up a 90 day payment plan. Please contact the clinic as soon as possible if you need to discuss this option. To avoid legal action with unpaid bills, be sure to contact our office as soon as possible if you are experiencing financial difficulty. Unpaid bills may be referred to an outside collection service as a last resort, which could affect your credit status.
Will the clinic continue to care for me if I have filed for bankruptcy or have had my account turned over to collections?
We invite patients with bankruptcy or collection status to seek future medical care elsewhere. When the required bankruptcy period has passed or when the collection amount has been paid, these patients are welcome to return to the clinic.
What if I have a complaint, a compliment, or I want to make a suggestion about my experience at the clinic?
Your suggestions and comments about our clinic are welcome. You may call our practice manager at the clinic, or you may complete the online Patient Satisfaction Survey.
I need to ask my doctor a quick question. What is the best way to contact him/her?
The best way to communicate with your doctor is to call the clinic and leave a message with your doctor’s physician or medical assistant. You may also complete the online “Contact Us” form and we’ll get back to you as soon as possible. Please remember: do not include any personally identifiable health data in your message.
How can I get a copy of my medical records?
You can request a copy of your medical records by signing an Authorization to Release Medical Records and Information Form available online or by calling us at 801-568-3480. Sign and send the form to our clinic at 3584 West 9000 south, suite 405, West Jordan, UT 84088, or fax at 801-568-3482. Please indicate if you want to pick up your records or have them mailed to you.
Records needed for continuing care purposes will be sent directly to your provider.

How do I get my test results?
When your doctor orders diagnostic tests for you, our office will usually receive the results within a week. Your doctor will discuss your test results with you either by telephone or at your next appointment. We will communicate with you by letter or by telephone, so please be sure that your name, address, and telephone number are up to date in our records.
